Making an Appointment
Sessions are by Appointment only. Please fill out the form below and include why you are interested in the service chosen, time frame of sustaining injury, where you heard about our business, if assistance is required to go up or down stairs, allergies to animals, when you would like to schedule an appointment and the best time to call.
On the Appointment Day
For your first session, you will be asked to complete a Health History and Consent Form.
In fairness to clients who are on time, we must remain on schedule. If you are late, we will work on you for the remainder of your scheduled time, however the full session fee will still be due.
Purchasing Product
If you are interested in purchasing a product from us please include the product name, size and quantity in the comment box. We will contact you with a confirmation and total price.
Payment Policy
Full Payment is due at the end of each session and before your products are delivered, shipped or picked up. See Services & Specials or The Attic Shoppe for pricing. We accept cash or local checks only.
